Make a Report

We encourage all members of the UCSF community—patients, staff, faculty, and students—to report incidents involving sexual violence, sexual harassment, discrimination, and harassment to OPHD. Reporting helps us offer support and ensure that all members of the UCSF community have a safe and inclusive learning and working environment.

Effective, Monday, August 5, 2024: All new concerns about unwelcome conduct, discrimination, or harassment regarding antisemitism, Islamophobia, anti-Jew or Israeli hate, or anti-Arab or Palestinian hate, will be temporarily assigned for initial assessment to the Office of Ethics and Compliance, Investigations Unit (IU).

How to report

Reports can be filed by:

  • Online: Fill out OPHD’s online Reporting Form to file a Discrimination/Harassment complaint.
  • Voicemail: (415) 502-3400 (leave a message to request a callback). We will return your call as soon as possible.

We do not provide emergency services. If you are experiencing an emergency or need immediate assistance with sexual assault, stalking, relationship violence, or other crimes, please contact the UC San Francisco Police Department by calling  911 or (415) 476-6911.

Immediate Access to Support Services

If you need immediate support services and/or are unsure about reporting, we recommend contacting one of the following confidential resources for information and support:

  • Student Mental Health and Wellbeing (SMHW) is a resource for UCSF students navigating personal concerns, identifying options, reducing stress, and solving problems. SMHW offers confidential counseling and psychiatry services to registered students. Several mental health options can be found on the SMHW brief consult page. During business hours, students can also call (415) 476-1281 to schedule an appointment. For after-hours support including weekends, press option 2 to speak with the Consult Line.
  • Faculty and Staff Assistance Program (FSAP) provides confidential counseling services for personal and work-related issues to faculty and staff. Support is offered for a wide range of issues including work related stress, relationship/couple issues, family problems, depression, anxiety, or other emotional problems, grief and loss, alcohol, drug and other addictions, consultations and training services for faculty and staff management. The FSAP crisis line can be reached at (415) 476-8279.
  • Campus Advocacy, Resources and Education (CARE) provides free, confidential support to any UCSF affiliate, including learners, staff, and faculty who have experienced interpersonal violence, sexual assault, dating/intimate partner violence, sexual harassment, or stalking. CARE can be reached at [email protected] or 415-502-8802 (available 24 hours for emergencies, press 1 to reach a crisis counselor after normal business hours).
To file a report anonymously
  • File an anonymous online complaint at the UC EthicsPoint website.
    • Choose "Report a Concern Online"
    • Enter a campus location for "Where Did This Occur?"
    • Read the important reminder about passwords and hit" Continue."
    • Next select "Discrimination/Harassment" or "Sexual Misconduct" for the Topic Which Best Describes Your Concern.
    • You will need to agree to the terms and conditions and choose a password before submitting the Report.
  • File an anonymous phone complaint by calling the EthicsPoint Hotline at (800) 403-4744.

Your anonymous report will be routed to OPHD.

What should I include in my report to OPHD?
  • Names of all individuals involved and their relationship to UCSF (student, patient, employee, third party)
  • Your contact information and your relationship to UCSF (student, patient, employee, third party)
  • What occurred, where it happened, and when the incident occurred (if known)
  • Type of alleged harassment or discrimination (e.g., race, gender, religion, disability, etc.)
What happens after I make a report?

OPHD will review your report to determine if the alleged conduct is covered by the Sexual Violence and Sexual Harassment Policy and/or the Anti-Discrimination Policy. If the concern falls under these policies, a Complaint Resolution Officer (CRO) will contact you to schedule a meeting to discuss your concerns. During the meeting, the CRO will provide information about support resources and discuss options for next steps. If the reported behavior is not within OPHD’s scope, we will contact you and direct you to the appropriate UCSF office or resource.
